As a hybrid provider of care, we offer multiple options to best take care of you.
At Fortify, you can elect the Traditional Insurance route or choose a Cash Based Care route. Both can be great options depending on your situation.
Insurance Based Physical Therapy

In our insurance-based model, patients have the option to utilize their insurance coverage for our services.
This is sometimes subject to regulation from insurance companies who may limit the frequency, duration, or types of treatment you can receive.
This is often easiest and cheapest when we are “In-network” with your insurance company.
Price per visit will vary both on your specific insurance and the type and amount of treatment your receive each visit.

Direct Billing: If the healthcare provider is in-network with your insurance, they will bill your insurance company directly for the services provided. This simplifies the billing process for you, as the patient.
Out-of-Pocket Costs: Despite the direct billing, depending on your specific insurance plan, you may still be responsible for certain out-of-pocket expenses such as co-pays (a fixed amount for a service), co-insurance (a percentage of the allowed amount for the service), or deductibles (the amount you pay for covered health care services before your insurance plan starts to pay).
Coverage Details: To understand what your insurance plan covers, including specific services like physical therapy, it's recommended to contact your insurance company directly. This can typically be done by calling the customer service number found on the back of your insurance card.

If we are not in-network with your insurance company, the process works differently:
Out of pocket costs: Out of pocket cost is higher initially, because you will pay the full cost of your therapy each time your receive treatment. You will then file with your insurance company who will pay you back a portion of what you paid for therapy.
Coverage Varies: How much your insurance will pay for out of network services varies between insurance companies. In order for you to file with your insurance company for re-imbursement, we will provide you with a “super bill” which provides itemized charges for each visit. It is then your responsibility to file that claim with your insurance company. You may have a separate out of network deductible.
Cash Based Care

Cash Based Care is a way to receive high quality, flexible, and personalized care at Fortify without involving the insurance companies.
It is an uncomplicated exchange of service and payment. This allows the patient and therapist to have greater control over their care with maximal flexibility.
Payment is collected at the time of service.

Limitless: There are no limits of frequency of visits or the number of visits a patient can be seen for, unlike insurance based models. Come as often as you want for as long as you need to.
Price Consistency: Because you are being charged per treatment session instead of based on what is done during a visit, the cost of each visit is the same. This allows for easier budgeting.

Initial evaluation:
Evaluation and initial treatment: $150
Return visits after evaluation:
Including whatever treatments are best for the patient for as long as they need: $120/visit
